This form is for the named person's use only. EPE Requires 24-48 hours notice for all requests.
This is not an approval. Agreement must be signed. This is not to be considered legally binding as an approval, permission,
or authorization to use or represent patents, marks, trademarks and/or
copyrighted material owned or licensed by EPE.
All filming will be escorted on property. Subject to cancellation without notice. Strict filming / photography max of 2 hours, no exceptions and limitations provided by EPE. Insurance must be provided.
